
Providing a clean, safe environment for you and your service is our #1 priority. Our staff already followed strict cleaning and disinfecting protocols regulated by local agencies before the effects of COVID-19, but we’ve now adopted extra safety precautions and placed even greater emphasis on deep cleaning. We remain committed to continually evaluating what must be done to meet the needs of our customers and adhere to (and exceed) both Centers for Disease Control (CDC) guidelines and state regulations.
The following safety precautions are being taken to protect the health and wellbeing of you and our staff during your visit:
*Our staff has taken a refresher course on Salon/Spa Infection Control and earned certificates
*Our staff will continue to wash their hands thoroughly before and after contact with every client.
*Our spa uses an EPA-registered disinfectant on tools, containers, and all surfaces that you may come in contact with during your visit.
*Chlorine bleach is used for cleaning the salon.
*Please remain in your vehicle when you arrive for your appointment. We will send a text message when you are able to come into the spa for your appointment.
*Hand sanitizer will be available for clients and staff in every room.
*Staff will be wearing face mask during your services until further notice.
A new set of gloves will be used for every client and at times must be changed during the service, until further notice.
We ask that you please stay home if you’re feeling sick, and the same goes for our staff. We will be happy to work with you to schedule another appointment as soon as you are feeling better. We are working hard to keep you and the spa community safe, and we know we can count on you too. Together we will make it through this trying time. -The therapist retains the right to refuse service to any suspicious persons, those appearing to be under the influence of any drugs or alcohol, or those appearing to have signs and symptoms of any illness or disorder that may contraindicate the treatment to be given. Please be respectful of the health of others and reschedule if you are sick or have knowingly been in contact with any contagious conditions!
-Clients must give at least 24 hours notice of appointment cancellation and/or changes in desired service(s) in order to avoid a fee of up to 100% of the scheduled service amount at the discretion of the therapist. Please notify the therapist with full and honest disclosure as far in advance as possible when needing to change/cancel appointments. Confirmation texts/calls from the therapist are NOT required, but are merely done out of courtesy when time allows. Clients are expected to attend their appointments regardless of confirmation from the therapist. Please properly record your appointments on your calendar(s) upon scheduling, and feel free to verify with the therapist at any time. Last minute meetings, travel plans, work/school functions, etc. will not necessarily be exceptions from the cancellation fee. Please plan accordingly.
-No-Show clients, clients cancelling less than 24 hours before their appointment, and clients cancelling more than two consecutive times may be billed for the entire service and/or required to pre-pay in order to be rescheduled.
-Arriving late for an appointment may result in a shorter session at full-price.
-All fees are subject to change. Client should always verify prices before beginning a service.
-All services and products purchased, including gift certificates, are non-exchangeable & non-refundable.
-Minors under the age of 18 receiving a service must be accompanied by a parent or legal guardian at all times. No children are otherwise allowed during treatment sessions.
-Clients should be prepared to make their session as relaxing as possible by muting all cell phones and electronics, as well as informing the therapist of any special accommodations needed, and clarifying all questions and concerns.
-Clients should arrive hygienically prepared for their session.
-All clients must complete and sign all required forms before services can begin.
-All clients must inform the therapist at the start of every visit if they are under any new prescribed medical or over-the-counter oral or topical treatments, or have been diagnosed with any new illnesses or injuries. Medical clearance may be required before certain services can be performed.
-Clients should agree to follow therapist’s advice for post-session care to achieve the best treatment results.
-The therapist is not responsible for lost items or items left behind.
-Any sexual advances, sounds, comments or innuendos, or rude or violent behavior will result in immediate dismissal and will be reported to legal authorities.
Your spa treatments are reserved especially for you. We value your business and ask that you respect the spa’s scheduling policies. Should you need to cancel or reschedule, please notify us at least 24 hours in advance.
Any cancellations with less than 24 hours of notice are subject to a cancelation fee amounting to the cost of the scheduled service. Clients who miss their appointments without giving any prior notification will be charged in full for the scheduled service. We recognize the time of our clients and staff is valuable and have implemented this policy for this reason. When you miss an appointment with us, we not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time. Additionally, many times our staff will be functioning in an “on call” status and can have travelled to the spa specifically for your service. For these reasons we are obligated to compensate our staff for their time as well as make up for the lost revenue.
When you schedule your appointment with us, you are agreeing to these policies. All services require a credit card or gift certificate to guarantee a reservation so please have your credit card and/or gift certificate ready when booking. You will not be billed unless there is a cancellation or no show. Upon checkout, guests may choose their method of payment.

At Realm Spa & Cosmetics, we pride ourselves in excellence in client service. As part of our service, we comply with the Health Insurance Portability and Accountability (HIPAA) Act of 1996. This Notice of Privacy Policy and HIPAA Compliance describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully before signing below.
Uses and Disclosures of Your Personal/Health/Medical Information
For Treatment: We may use medical information about you to provide you with treatment or services. For Payment: We may use and disclose medical information about you so that the treatment or services you receive may be billed to and payment collected from you, an insurance company or a third party. As Required By Law: We will disclose medical information about you when required to do so by federal, state or local law. For Military and Veterans: If you are a member of the armed forces, we may release medical information about you as required by military command authorities. For Worker’s Compensation: We may release medical information about you for worker’s compensation or similar programs. For Lawsuits and Disputes: If you are involved in a lawsuit or a dispute, we may disclose medical information about you in response to a court or administrative order. For National Security and Intelligence: We may release medical information about you to authorized federal officials for intelligence, counterintelligence and other national security activities authorized by law.
Your Rights Regarding Your Personal/Health/Medical Information
Your Right to Inspect and Copy: To inspect and request a copy of your medical information, you must submit your request in writing. Your Right to Amend: If you feel the medical information we have about you is incorrect or incomplete, you may request an amendment in writing. Your request may be denied if you do not include a reason to support your request. Your Right to an Accounting of Disclosures: You have the right to request in writing, a list accounting for any disclosures of your medical information we have made. Your Right to Request Restrictions: You have the right to request a restriction or limitation on the medical information we use or disclose about you for treatment or service and collection of payment. Your Right to Request Communications: You have the right to request how you are communicated with regarding appointments, reminders, payments, discounts or specials, follow ups with a signed Contact Consent Form (separate form). Your Rights to a Paper Copy of This Notice: You have the right to a paper copy of this notice at any time.
Contacting the Privacy Officer
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer: Chasity Skidmore or with the Secretary of the Department of Health and Human Services.
Changes to This Privacy Policy
We reserve the right to revise or amend this Privacy Policy at any time. We will provide you with a notice of any revisions or amendments to this policy or changes in the law affecting this policy, electronically within 60 days of the effective date of revision or amendments.
HIPAA Authorization Form
If you provide us permission with a signed HIPAA Authorization Form (separate form), can we use or disclose medical information about you. You may revoke that permission, in writing, at any time. If you revoke your permission, we will no longer use or disclose medical information about you for the reasons authorized. You understand that we are unable to take back any disclosures we have already made prior to revocation. We are required to retain our records of the care we provided to you for 3 years.
CHEMICAL PEEL: 7-10 DAYS BEFORE YOUR APPOINTMENT
• Inform your clinician if you have taken Accutane (oral acne medication) in the past year. • Cease application of all topical retinoids, or Vitamin A derivative products. • Cease use of all Glycolic or chemical exfoliants. • Cease use of all mechanical exfoliants, such as scrubs. • Avoid facial waxing. • Avoid sun exposure without a sunscreen of SPF 30 or greater. Avoid tanning beds.
THE DAY OF YOUR APPOINTMENT • Allow 60 minutes for your appointment. • Pre-Treatment photos will be taken.
CHEMICAL PEEL: WHAT TO DO AFTER YOUR TREATMENT • Expect your skin to appear pink for a few hours after treatment. • Your skin may be temperature sensitive for a few days after treatment. • You will be given a cool compress to apply after treatment if required. • Make-up may be applied to cover redness as needed. • Avoid direct sunlight on the treated area and use a broad spectrum UVA/UVB sunscreen of SPF 30 or greater for 1-2 weeks after your treatment. • Keep your skin well hydrated with a protective moisturizer, • Avoid using abrasive products such as scrubs or exfoliating sponges for 7 -10 days. • Avoid products such as retinols and glycolic acid 10 days. • Vitamin C Serums may be resumed after 48 hours.
WHAT TO EXPECT • Slight peeling for 3-5 days. • Mild Itching, irritation, redness, increased sensitivity. • Small whiteheads. • Crusting is rare; however if it does occur, a thin layer of over-the-counter antibiotic ointment such as Polysporin® can be applied 2-3 times a day for 3 days. Please call us if you experience this uncommon reaction. • Hyperpigmentation (darkening of the skin color) can occur in some skin types. Please call us if you experience this uncommon reaction.
MAXIMIZING BENEFITS OF YOUR TREATMENT • Our Skin Care Specialists will review and compose a simple, customized skin care program for you to begin before your treatment as well as once your skin has healed to maintain the desired results. A quality, daily home skin care regime will ensure that you maximize and maintain the benefits of your treatment.
What is Dermaplaning?
Dermaplaning is a form of manual exfoliation similar in theory to microdermabrasion but without the use of suction or
abrasive crystals. An esthetician grade, sterile blade is stroked along the skin at an angle to gently “shave off” dead
skin cells from the epidermis. Dermaplaning also temporarily removes the fine vellus hair of the face, leaving a very
smooth surface.
As with any type of exfoliation, the removal of dead skin cells allows home care products to be more effective,
reduces the appearance of fine lines, evens skin tone and assists in reducing milia, closed and open comedones, and
minor breakouts associated with congested pores.
Dermaplaning can be an effective exfoliation method for clients that have couperose (tiny blood vessels near the
surface of the skin), sensitive skin or allergies that prevent the use of microdermabrasion or chemical peels.
Due to the contours of the face, certain areas of the face (such as the eyelids and nose) are not treatable using this
method.
What should you expect during your treatment?
As your esthetician, I will perform a thorough skin analysis prior to your first dermaplaning.
If dermaplaning is not appropriate, you will be informed during this session and an alternative treatment may be
recommended instead.
If dermaplaning is not contraindicated, maximum results are obtained by participating in a series of treatments plus
following a home care regimen.
I will review your current daily regimen and skin care products, advise you on which products you should continue to
use, and recommend any additional products or changes to your regimen to enhance your desired outcome.
As your esthetician, I take every precaution to ensure that your skin is well hydrated and calm following each session.
However, you may experience excessive dryness or even some peeling between sessions, which may or may not be
normal. Always contact me if you have any concerns.
More sensitive skin may experience some redness after the first couple of sessions. This normally goes away after 2
to 3 hours. Dermaplaning may cause minor superficial abrasions which may not appear until a day or two following
your treatment. If this should occur, please contact me so that I can do a post-treatment follow up with you.
After your treatment, SPF 30+ MUST be worn at all times. Tanning beds should never be used. You are making an
investment in your skin: therefore, it is to your benefit to continue to protect it long after your series of treatments is
completed.
Is satisfaction guaranteed?
The majority of my clients receive noticeable, satisfactory to above average results with a series of treatments and
a commitment to a daily skin care regimen. However, this outcome cannot be guaranteed as maximum results
are highly dependent on age, cumulative sun exposure, health, lifestyle, genetic traits, general skin condition, and
willingness to follow recommended protocols.
Be aware that many changes may occur deeper within the skin over time. To continue the maintenance of your skin
after you complete your treatment(s), I may inform you of long-term age management programs.
Contraindications
Although it is impossible to list every potential risk and complication, the following conditions are recognized as
contraindications for dermaplaning treatment and must be disclosed prior to treatment.
• Active acne
• Active infection of any type, such as herpes simplex or flat warts.
• Any raised lesions
• Any recent chemical peel procedure
• Chemotherapy or radiation
• Eczema or dermatitis
• Family history of hypertrophic scarring or keloid formation
• Hemophilia
• Hormonal therapy that produces thick pigmentation
• Moles
• Oral blood thinner medications
• Pregnancy
• Recent use of topical agents such as glycolic acids, alpha-hydroxy acids and Retin-A
• Rosacea
• Scleroderma
• Skin Cancer
• Sunburn
• Tattoos
• Telangiectasia/erythema may be worsened or brought out by exfoliation
• Thick, dark facial hair
• Uncontrolled diabetes
• Use of Accutane within the last year
• Vascular lesions
Post-Treatment/Home Care
Aerobic exercise or vigorous physical activity should be avoided until all redness has subsided. Direct sunlight
exposure is to be completely avoided immediately following the treatment (including any strong UV light exposure or
tanning beds). Although SPF 30+ should already be a part of your daily skin care, after dermaplaning, SPF 30+ must
be applied daily to the treated area for a minimum of two weeks. Twice daily cleanse the treated area with a post treatment cleanser, followed by a serum or treatment cream and follow with SPF 30+ sunscreen.
